Standard Form 1409 (SF 1409): Abstract of Offers

Standard Form 1409 (SF 1409), Abstract of Offers, is a one-page sheet. The footer says it is prescribed by GSA under FAR (48 CFR) 53.214(f), and GSA hosts it in its forms library. The edition printed on the form is STANDARD FORM 1409 (REV. 10/2023), and the footer says it is authorized for local reproduction and the previous edition is usable.

The sheet records the offers received in response to a solicitation: the solicitation number, opening date, issuing office and a general description of the supplies or services, then a line for each offeror with its acceptance time, business size, prices under three item columns, delivery time, F.O.B. point and remarks.

It ends with a certification that the signer opened, read and recorded on the Abstract, and on Continuation Sheets if any, all offers received in response to the solicitation. You can fill it out free in your browser on this page and download the result without an account.
