Abstract of Offers - Construction - Continuation Sheet
Optional Form 1419A (OF 1419A), Abstract of Offers - Construction Continuation Sheet, is a one-page sheet that extends OF 1419, Abstract of Offers - Construction. GSA hosts it in its forms library. The edition printed on the form is OPTIONAL FORM 1419A (11-88), and the footer says the form is authorized for local reproduction and the previous edition is usable.
Its instructions say to attach it to OF 1419 when more than 2 offers are received on a construction project. Each continuation sheet accommodates 13 contract items, to conform to the number of items that can be entered on OF 1419, and has four offer columns with each offeror's bid security, amendments acknowledged, unit prices and estimated amounts.
For contract items in excess of 13, the instructions say to use additional OF 1419s and attach additional continuation sheets as needed. You can fill it out free in your browser on this page and download the result without an account.
