GSA Form 1380: National Security Position

GSA Form 1380, National Security Position, is a General Services Administration form, REV. 3-99 in its footer. The footer says it is prescribed by ADM P 9732.1 C, while GSA's page lists ADM P 9732.1B as its authority. GSA hosts it in its forms library.

The form says it is used to request security approval to fill a national security position. Part A names the candidate, Part B describes the position and its sensitivity level, Part C gives funds information, and Part D holds the requester's certification. The form ends with two signatures: the requester and the official who authorizes the request.

The top of the form says the candidate must submit five investigative forms: SF 86 (original and copy), SF 87 (two sets, not required for periodic reinvestigations), SF 312 (original only, if requesting National Security Clearance), an employee application (one copy) and OF 306 (original copy). You can fill it out free in your browser on this page and download the result without an account.
