Schedule J-2: Expenses for Separate Household of Debtor 2 (individuals)
Form B 106J-2, Schedule J-2: Expenses for Separate Household of Debtor 2, is the Official Bankruptcy Form for Debtor 2's own household expenses in a joint bankruptcy case. The U.S. Courts list it under Individual Debtors. The current version is dated 12/15, and the U.S. Courts page lists it as effective on December 1, 2015. The page says the form is derived from Official Form 6J, and that Official Bankruptcy Forms are approved by the Judicial Conference and must be used under Bankruptcy Rule 9009. The committee note says the form is new and is used to report the monthly expenses of Debtor 2 in a joint debtor case only if Debtor 1 and Debtor 2 maintain separate households.
The 3-page form follows Schedule J line by line: the household and dependents, then monthly expenses from housing and utilities to insurance, taxes, car payments and support. Line 22 gives the monthly expenses of Debtor 2, which the form says to copy to line 22b of Schedule J. Line 23 is not used on this form.
This page explains the form in plain words and is not legal advice. You can fill it out free in your browser on this page and download the result without an account.

The form says to answer only for Debtor 2's expenses that are not reported on Schedule J. The booklet says to show all totals as monthly payments and to write $0 for a line with nothing to report.

With the United States Bankruptcy Court for the district named at the top of the form. The booklet says that if you are filing jointly and Debtor 1 and Debtor 2 keep separate households, Debtor 2 must complete and include Schedule J-2.
The form does not print a fee.
The instruction booklet lists Schedule J (Official Form 106J) among the forms filed either when the bankruptcy case is filed or within 14 days after the Voluntary Petition (Official Form 101), and says the case may be dismissed if they are not. It does not list Schedule J-2 separately.
The form says not to state the dependents' names. The booklet says filing a bankruptcy case is not private and anyone has a right to see the forms after they are filed, unless the court orders otherwise under 11 U.S.C. § 107.
