AO 435
Transcript Order
AO 435, Transcript Order, is a form from the Administrative Office of the United States Courts. The form says to use it to order the transcription of proceedings, with a separate order form for each case number. The PDF prints Rev. 10/23, and the U.S. Courts page, which lists it under Other Forms, shows it as effective on April 1, 2018.
Items 1 to 19 ask for the person ordering, the case, the dates and location of the proceedings, what the order is for, the portions of the proceedings requested, the delivery category and copies, and a signed certification to pay all charges. Shaded areas, including the due date and the order tracking lines, are reserved for the court's use. Page 2 holds the form's instructions.
This page explains the form in plain words and is not legal advice. You can fill it out free in your browser on this page and download the result without an account.

Name, phone number, date, delivery address or email, city, state and ZIP code.
Case number, judge, dates of proceedings from and to, case name, and the city and state where the proceedings took place.

Boxes for appeal, non-appeal, criminal, civil, Criminal Justice Act, in forma pauperis, bankruptcy and other (specify).

Portions with dates: voir dire, opening statements and closing arguments for plaintiff and defendant, opinion of court, jury instructions, sentencing, bail hearing, testimony (specify witness), pre-trial proceeding and other.

Delivery category (30-Day, 14-Day, 7-Day, 3-Day, Next-Day, 2-Hour or Realtime), original, first copy and number of additional copies. The page estimate, costs and estimate total are shaded for the court.
A signature certifying payment of all charges (deposit plus additional) and the date. The form lists distribution as court copy, transcription copy, order receipt and order copy.
The instructions say to complete a separate order form for each case number, and that only one case number may be listed per order in Item 8.
The instructions say Items 1 to 19 should always be completed, and not to complete the shaded areas, which are reserved for the court's use.
Place an X in each Item 15 box that applies and in the Item 16 box for each portion, and list the specific dates. The form says orders may be placed for as few pages of transcript as are needed.
In Item 17, mark a category and the original or copies. The form says an original must be ordered and prepared before copies are available, and its fee includes the copy for the records of the court.
Item 18 is signed to certify that all charges will be paid, including the deposit plus any additional charges. Item 19 is the date of signing.
The instructions say to submit the form in the format required by the court and to keep a copy for your records.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The form says to submit it in the format required by the court. It prints no address of its own; the court address is a line in the shaded area.
The form does not print a fee amount. It says the court will notify you of the amount of the required deposit fee, which may be mailed or delivered to the court, and the court processes the order upon receipt of the deposit.
Delivery time is computed from the date of receipt of the deposit fee, or for transcripts ordered by the federal government, from the date of receipt of the signed order form. A 30-Day transcript is delivered within thirty (30) calendar days after receipt of an order.
The form says full price may be charged only if the transcript is delivered within the required time frame. A 7-day order not delivered within seven (7) calendar days is paid at the 14-day rate, and if not delivered within 14 calendar days, at the 30-day rate.
If the deposit fee does not cover all charges, the form says the court will notify you of the balance due, which must be paid prior to receiving the completed order. The court notifies you when the transcript is completed.
