AO 436
Audio Recording Order
AO 436, Audio Recording Order, is a form from the Administrative Office of the United States Courts for ordering duplicate audio recordings of court proceedings. The U.S. Courts list it under Other Forms. The current version is Rev. 04/13, and the U.S. Courts page lists it as effective on April 1, 2013.
The order page asks for the ordering party's contact details, the case number and name, the dates and location of the proceedings, the presiding judge, what the order is for, the portions of the proceedings requested, and the format and number of copies. The instructions on page 2 say to complete a separate order form for each case number and not to complete the shaded areas, which are reserved for the court's use.
This page explains the form in plain words and is not legal advice. You can fill it out free in your browser on this page and download the result without an account.

The instructions say that for orders of 20 or more audio recordings, the court will notify you of the amount of the required deposit fee, which may be mailed or delivered to the court, and that the court processes the order on receipt of the deposit.
The instructions say delivery time is computed from the date the deposit fee is received, if one was requested, and otherwise from the court's receipt date. The court notifies you when the recordings are completed.
If the deposit did not cover all charges, the instructions say the court will notify you of the balance due, which is paid before the completed order is received.
