AO 44: Invoice

AO 44, Invoice, is a United States District Court form for billing transcript charges. The U.S. Courts list it under Court Reporter Forms. The current version is Rev. 04/18, and the U.S. Courts page lists it as effective on April 1, 2018.

The invoice names who it is sent to and who the check is made payable to, whether the case is criminal or civil, the case number and title, the dates the transcript was ordered and delivered, and the charges for the original, first copy and additional copies in each delivery category. It ends with a certification signed by the official court reporter. It is completed by the court reporter, not a form members of the public file.

This page explains the form in plain words and is not legal advice. You can fill it out free in your browser on this page and download the result without an account.
