Part 1: Filing type
Initial or additional filing. An additional filing gives the receipt number of the most recent Form I-956H and says whether the answers to Parts 3, 4 and 5 are the same as before.
I-956H
Bona Fides of Persons Involved with Regional Center Program
Form I-956H, Bona Fides of Persons Involved with Regional Center Program, is how each person involved with an EB-5 regional center, new commercial enterprise (NCE) or affiliated job-creating entity (JCE) attests to their eligibility to be involved with that entity. The instructions say a person is involved if they are, directly or indirectly, in a position of substantive authority to make operational or managerial decisions over pooling, securitization, investment, release, acceptance, or control or use of any funding.
The USCIS page says Form I-956H is a distinct filing submitted along with Form I-956, Application for Regional Center Designation, or Form I-956F, Application for Approval of an Investment in a Commercial Enterprise, or when persons otherwise attest to their compliance with section 203(b)(5)(H) of the Immigration and Nationality Act.
The page also says not to submit Form I-956H as an exhibit to Form I-956 or Form I-956F, or as an exhibit in a response to a USCIS notice.
Initial or additional filing. An additional filing gives the receipt number of the most recent Form I-956H and says whether the answers to Parts 3, 4 and 5 are the same as before.
Tables for the regional center, NCE, affiliated JCE and non-affiliated JCE with each entity's name, ID number and other names, then each role held, such as owner with a percentage of ownership or director with a title.
For an individual: names, date and country of birth, citizenship, passports, U.S. national or permanent resident status, A-Number and Social Security number. For an organization: name, date and place established and EIN. Then mailing address and contact details.
Thirteen Yes or No questions on fraud or deceit offenses, prison sentences, final orders of financial regulators, controlled substances, espionage, money laundering, terrorism, human trafficking, foreign asset control and professional discipline.
Five Yes or No questions on rescission or removal proceedings, foreign government entities, capital from a foreign government source, and ownership by a foreign sovereign wealth fund or state-owned enterprise.
The statement, certification and signature of the person involved or the authorized individual, the interpreter and preparer sections, and additional information space.
Type or print legibly in black ink. The USCIS page says every page has to come from the same edition, with the edition date and page numbers visible, or USCIS may reject the form.
The instructions say to select initial filing if no Form I-956H was filed before, and additional filing if one was, even for a different entity. Parts marked as unchanged are incorporated by reference and may be skipped.
In Part 2, complete the rows for the entities you are involved with and every role you hold. An Other role is described in Part 9 if more space is needed.
The form says to answer Yes to any question that applies, even if records were sealed or cleared and wherever in the world it occurred, then explain each Yes in Part 9 with documentation and the outcome.
Write N/A where a question does not apply and None for a zero. Extra sheets carry the name at the top, the page, part and item number, and a signature and date.
The person involved, or the authorized individual for an organization, signs and dates it. A stamped or typewritten name is not accepted. This page lets you fill the form online free, then print it for signature.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The USCIS page says each Form I-956H goes in a separate envelope or folder in the Form I-956 or Form I-956F packet, identified in a cover letter or memo, and the packet is mailed to the address on that form's own page.
The I-956H page lists a U.S. Postal Service address (USCIS, Attn: I-956H, P.O. Box 660168, Dallas, TX) and a Lewisville, TX address for FedEx, UPS and DHL deliveries. It lists no online filing option.
For a Request for Evidence or Notice of Intent to Deny, the page says to mail Form I-956H to the I-956H address, not as an exhibit, and write the notice's receipt number in Part 9. The rest of the response goes where the notice says.
The fee is on the USCIS Fee Schedule page (Form G-1055), which can change. Check it on the day you file. The page says USCIS rejects a form not filed with the appropriate fee.
The page checklist and the instructions list a copy of the front and back of the Permanent Resident Card for a lawful permanent resident.
Send legible photocopies unless originals are requested, and include a full English translation with the translator's certification for any foreign-language document. The page says USCIS rejects an unsigned form.

The instructions say a person who is, directly or indirectly, in a position of substantive authority to make operational or managerial decisions over pooling, securitization, investment, release, acceptance, or control or use of any funding.
Yes. The instructions say each person completes a Form I-956H that identifies each entity they are involved with, and the page says each one goes in a separate envelope or folder in the Form I-956 or Form I-956F packet.
No. The page says not to submit it as an exhibit to Form I-956 or Form I-956F or to a response to a USCIS notice, and that doing so may lead to rejection of the associated form and delay.
Select additional filing in Part 1 and give the most recent receipt number. Where the Part 3, 4 or 5 answers are unchanged, the instructions say they are incorporated by reference and those items may be skipped.
The I-956H page lists only mailing addresses: one for U.S. Postal Service deliveries and one for FedEx, UPS and DHL.
USCIS fees change, so this page does not list one. The I-956H page points to the USCIS Fee Schedule, and the instructions point to Form G-1055.
