I-102
Application for Replacement/Initial Nonimmigrant Arrival-Departure Document
Form I-102, Application for Replacement/Initial Nonimmigrant Arrival-Departure Document, is the USCIS form a nonimmigrant uses to request a new or replacement of an incorrect, lost, stolen or mutilated arrival-departure document. The instructions say this covers Form I-94, Arrival/Departure Record, Form I-94W, Nonimmigrant Visa Waiver Arrival/Departure Record, and Form I-95, Crewman's Landing Permit.
The USCIS page separates errors made by CBP from errors made by USCIS. If U.S. Customs and Border Protection (CBP) issued the record with incorrect information, such as a misspelled name or wrong date of admission, the page says not to file Form I-102 and to go to the nearest CBP port of entry or deferred inspection office instead. For an electronic Form I-94 from an air or sea port, the page points to CBP's Form I-94 webpage for a paper copy, and the instructions say Form I-102 can be used if the record cannot be obtained there.
A-Number, USCIS online account number, legal and other names, U.S. mailing and physical addresses, date and country of birth, citizenship and Social Security number.
Date, place and type of port of last entry, class of admission, current status and expiration date, the record number, passport or travel document details, and the name exactly as it appears on the record.
One box from 1.a. to 1.g.: a lost or stolen record, a mutilated record, no Form I-94 at admission, a USCIS-issued record with an error, or an initial Form I-94 for a military member.
Whether the application is filed with any other petition or application, and whether the applicant is now in removal proceedings.
Phone numbers, email, the applicant's certification under penalty of perjury and the signature.
Sections for anyone who interpreted or prepared the application, plus extra space with page, part and item numbers.
The USCIS page says a CBP-issued error is corrected at a CBP port of entry or deferred inspection office. The instructions say getting a paper copy of an electronic Form I-94 from the CBP website is free.
The instructions say to select only one box that matches the reason for filing. Each reason has its own evidence list.
Items 19.a. to 19.c. take the name exactly as shown on the record. If it differs from the current legal name, the instructions ask for evidence of the name change, such as a marriage certificate or court order.
Type or print in black ink. Write N/A where a question does not apply and None for a zero. Extra answers go in Part 7 with the page, part and item number.
The USCIS page says the form is rejected if these are missing: Family Name, U.S. Mailing Address and Date of Birth in Part 1, and the Reason for Application in Part 2.
A stamped or typewritten name is not accepted. A parent or legal guardian may sign for an applicant under 14. This page lets you fill the form online free, then print it for signature.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The USCIS page says to file with a USCIS lockbox, at the address on the Direct Filing Addresses for Form I-102 page for the reason for filing. USCIS changed the filing location on Oct. 7, 2026, and accepts forms postmarked on or before Nov. 9, 2026, sent to a prior location.
The fee is on the USCIS Fee Schedule page, and the instructions point to Form G-1055. Fees can change, so check it on the day you file.
The instructions ask for a copy of a government-issued ID verifying legal name and date of birth, with front and back copies of supporting documents. Originals are not sent unless instructed.
Lost or stolen: passport biographic and admission pages, and a police report or an explanation. Mutilated: the original damaged form. Not issued: the passport with the CBP admission stamp. USCIS error: the original record, a signed statement and proof of the correct information.
The instructions say a United States address is needed to file this application. Foreign-language documents need a full English translation with the translator's certification.
For an email or text when the lockbox accepts the form, the USCIS page says to complete Form G-1145 and clip it to the first page.

The instructions say a parent or legal guardian may sign for an applicant under 14. A legal guardian may also sign for a mentally incompetent person.
The instructions say only for a person admitted at a land border, or at an airport or seaport on or before June 28, 2010.
USCIS fees change, so this page does not list one. The I-102 page points to the USCIS Fee Schedule, and the instructions point to Form G-1055.
