Acknowledgement and General Information for Taxpayers Who File Returns Electronically
Form 9325, Acknowledgement and General Information for Taxpayers Who File Returns Electronically, is a notice for a taxpayer whose federal income tax return was filed electronically. It opens with "Thank you for participating in IRS e-file." The form carries its own "Instructions for Electronic Return Originators," and it says EROs can use the Acknowledgement File information, translated by the transmitter, to complete it. The taxpayer is the one who reads it.
Check boxes 1 to 6 record the tax year filed, the Submission Processing Center, the acceptance date and Submission ID, exception processing, whether an electronic funds withdrawal payment request was accepted, and whether a Form 4868 extension was accepted. The rest of the form explains changing a return, checking on a refund, refund offsets, paying a balance due and refund related financial products. It states: "DO NOT SEND A PAPER COPY OF YOUR RETURN TO THE IRS."
You can fill it out free in your browser on this page and download the result without an account.

Line by line rules for boxes 2 to 5, tied to the PIN Presence Indicator, the Acceptance Code and the Payment Acknowledgement Literal in the Acknowledgement File.
Enter the taxpayer's name, and the address if wanted. In box 1, enter the tax year, the Submission Processing Center and who provided the electronic filing services.
Check it if the taxpayer entered a PIN or authorized the ERO to enter or generate one, and the PIN Presence Indicator is "Practitioner PIN," "Self-Select PIN" or "Online Filer PIN." The instructions say Form 8879 is required if the ERO enters or generates the PIN or the Practitioner PIN method is used.
Check it if the Acceptance Code equals "Exception." The instructions say this means the return was rejected before and the later submission still has invalid data.
For an electronic funds withdrawal, check box 4 if the Payment Acknowledgement Literal equals "Payment Request Received," and box 5 if it does not. With box 5, the instructions say to tell the taxpayer to pay by check, money order, debit card, or credit card.
If a Form 4868 extension was accepted, enter its acceptance date and Submission ID.
These steps follow the Instructions for Electronic Return Originators printed on the form.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The form says: "DO NOT SEND A PAPER COPY OF YOUR RETURN TO THE IRS. IF YOU DO, IT WILL DELAY THE PROCESSING OF THE RETURN."
The IRS notifies the ERO when a return is accepted, usually within 48 hours. After more than three weeks without a refund, the form points to "Where's My Refund?" at www.irs.gov, TeleTax at 1-800-829-4477 and the Refund Hotline at 1-800-829-1954.
The form says to send a Form 1040X to the Submission Processing Center that processes paper returns for your area. The address is at www.irs.gov or by calling 1-800-829-1040.
Call 1-888-353-4537 about a payment's status, or to cancel a scheduled payment if the bank account information on the return changed. The form says cancellation requests must be received no later than 11:59 p.m. E.T. two business days prior to the scheduled payment date.
The form lists Form 1040-V for payments not made electronically and Form 9465 to request an installment agreement, which can be ordered at 1-800-TAX-FORM (1-800-829-3676).
Refunds can be offset for overdue taxes and, through the Treasury Offset Program, for certain non-tax debts. The form lists the Treasury Offset Program Call Center at 1-800-304-3107.
