U.S. Individual Income Tax Transmittal for an IRS e-file Return
Form 8453, U.S. Individual Income Tax Transmittal for an IRS e-file Return, is the cover sheet for paper documents that go with an individual income tax return filed electronically. The IRS About page says taxpayers and electronic return originators (EROs) use it to send any required paper forms or supporting documentation listed next to the checkboxes on the front of the form, and that it is used solely to transmit those forms.
The form prints "File this form only if you are attaching one or more of the following forms or supporting documents." The list includes Form 1098-C, Form 2848, Form 3115, Form 5713, Form 8283, Form 8332 and Form 8949. The form also says "Don't sign this form" and not to send Forms W-2, W-2G or 1099-R with it.
This edition is for the tax year January 1 to December 31, 2026. You can fill out the PDF free in your browser on this page and download it without an account.

Print or type the information. The instructions say the address must match the address on the electronically filed return. Do not abbreviate a foreign country name.
The instructions say to enter a box number only if the post office does not deliver mail to the home address.
Enter the SSN in the space provided. For a joint return, list the SSNs in the same order as the first names.
Check the box for each form or document you are attaching. The form says not to attach anything not shown next to the checkboxes.
For a decree or agreement in effect after 1984 and before 2009, the instructions list the pages a noncustodial parent attaches instead of Form 8332: cover page, the pages with the required terms, and the signature page.
Leave the form unsigned, as the form instructs, and gather the attachments behind it.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
