4506-T-EZ
Short Form Request for Individual Tax Return Transcript
Form 4506-T-EZ is a one-page request for an individual tax return transcript. The form says individuals can use it to request a tax return transcript for the current and the prior three years that includes most lines of the original tax return. The transcript will not show payments, penalty assessments, or adjustments made to the originally filed return.
The IRS About page says the transcript format partially masks personally identifiable information while financial data stays fully visible for tax preparation, tax representation or income verification. Because the taxpayer identification number is masked, the form has a Customer File Number field that prints on the transcript.
The form says it cannot be used by taxpayers who file Form 1040 on a fiscal tax year; they use Form 4506-T. The form also says transcripts are mailed only to the taxpayer's address of record. You can fill out Form 4506-T-EZ free in your browser on this page and download the result without an account.
The name shown first on the return and the first SSN or ITIN, then the spouse's name and second SSN or ITIN if it was a joint return.
The current name and address, including apartment, room, suite or inmate number, and the previous address shown on the last return filed if it is different.
An optional field of up to 10 numeric characters that prints on the transcript, which masks the taxpayer identification number.
The year or years of the return transcript requested. The form says most requests will be processed within 10 business days.
An attestation checkbox for the authority to sign, the signature and date of the taxpayer, a spouse's signature and date, and the phone number of the taxpayer on line 1a or 2a.
The instructions say to enter the first SSN or ITIN shown on the return, for example your SSN for a Form 1040 with Schedule C. They say to enter an EIN if the request relates to a business return.
Enter the current address, including a P.O. box if used. The instructions say someone currently incarcerated includes their inmate identification number, name and address here.
Enter the address on the last return filed if it differs from line 3. The instructions say that if the addresses differ and the address was not changed with the IRS, file Form 8822.
Optionally enter up to 10 numeric characters. The instructions say it should not contain an SSN; if an SSN or name is used, the transcript shows a generic "9999999999".
Enter each year of the return transcript requested, written as a year such as "2008" in the form's example.
Complete every applicable line first, check the attestation box, then sign and date. The instructions say to sign exactly as your name appeared on the original return, and also sign your current name if it changed.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
Mail or fax the form to the address for the state lived in when the return was filed. With more than one transcript and two different addresses, use the address of the most recent return.
The chart lists RAIVS Team, Stop 6716 AUSC, Austin, TX 73301 (fax 855-587-9604); RAIVS Team, Stop 6705 S-2, Kansas City, MO 64999 (fax 855-821-0094); and RAIVS Team, P.O. Box 9941, Mail Stop 6734, Ogden, UT 84409 (fax 855-298-1145).
The instructions say the IRS must receive Form 4506-T-EZ within 120 days of the date signed by the taxpayer or it will be rejected.
The instructions say the form will not be processed and will be returned if the box in the signature area is unchecked.
The form says that since July 2019 the IRS mails tax transcript requests only to the address of record.
