Form 15110: Additional Child Tax Credit Worksheet

Form 15110 is the Additional Child Tax Credit Worksheet. It goes with IRS notice CP08: the form says to complete it and send it back with the required supporting documents, and to refer to the CP08 notice for information on using the Document Upload Tool.

The worksheet asks for contact information and, for each dependent child claimed on the 2025 tax return, the child's name, Social Security number and whether the child meets all of the eligibility factors the form lists. The form says that if there is at least one qualifying child, the worksheet goes back with a completed Schedule 8812, Credits for Qualifying Children and Other Dependents.

Social Security number rule

The form says each child must have a Social Security number valid for employment issued before the return's due date, including extensions, unless the child was born and died in 2025.

Eight factors for a qualifying child for 2025, including being under age 17 at the end of the year, the relationship test, living with you more than half the year and being claimed as your dependent.

Child table

Five rows for each child's name and Social Security number, with Yes and No boxes for whether the child meets all the eligibility factors.

How many children qualify

A space for the count of qualifying children, with the note to send the worksheet and a completed Schedule 8812 if at least one child qualifies.

How to fill out Form 15110

  1. 1

    Contact information

Enter your name and Social Security number, and your spouse's if you filed a joint return. If your address changed, the form says to give the current address, call 800-829-0922 or visit www.irs.gov.

  2. 2

    List each child

For each dependent child claimed on your original 2025 return, enter the name as it appears on the Social Security card and the Social Security number. Additional children go on a separate paper.

  3. 3

    Answer Yes or No

For each child, check Yes or No for whether the child meets all the eligibility factors. The form says a child doesn't qualify with a No to any factor, and may qualify with Yes to all.

  4. 4

    Count qualifying children

Enter how many children qualify. With at least one, the form says to send the worksheet together with a completed Schedule 8812.

  5. 5

    Add supporting documents

Include the required supporting documents. For a child born and died in 2025, the form asks for a copy of the birth certificate or hospital records showing a live birth.
