Form 13614-C (ja): Intake/Interview and Quality Review Sheet, Japanese Version

Form 13614-C (ja) is the Japanese version of Form 13614-C, the Intake/Interview and Quality Review Sheet that the IRS Volunteer Income Tax Assistance (VITA) and Tax Counseling for the Elderly (TCE) program uses before a volunteer prepares a return. The PDF is printed in Japanese, with the title "採用/面接および品質審査シート", for taxpayers who would rather answer the intake questions in Japanese.

The edition is dated October 2025 (2025年10月) and its questions cover 2025. Its checklist says to complete pages 1 to 5 and to have tax documents such as Forms W-2, 1099, 1098 and 1095, Social Security cards or ITIN notices for everyone on the return, and photo identification such as a valid driver's license for you and your spouse. An IRS-certified volunteer fills in the columns marked for the volunteer.

The same PDF ends with Form 15080 (ja), a Japanese consent for Global Carry Forward of return data. The English edition is Form 13614-C. You can fill out this PDF free in your browser on this page and download it without an account.
