VA Form VA40-1330: Claim for Standard Government Headstone or Marker

VA Form VA40-1330 is the U.S. Department of Veterans Affairs Claim for Standard Government Headstone or Marker. Its General Information Sheet describes two benefits: a burial headstone or marker for the grave of an eligible deceased Veteran, and a memorial headstone or marker for an eligible deceased Veteran whose remains have not been recovered or identified, were buried at sea, donated to science, or cremated and the remains scattered. The form says VA will also provide the applicant with a Presidential Memorial Certificate if the Veteran is eligible for a headstone, marker, or medallion.

The PDF is five pages: the General Information Sheet, illustrations of the standard headstones and markers with inscription rules, the claim itself in blocks 1 to 35 on pages 3 and 4, and a page of available emblems of belief. The claim has signature blocks for the applicant, the business that accepts delivery and the cemetery official. You can fill out the PDF free in your browser on this page and download the result without an account.
