Application for Pre-Need Determination of Eligibility for Burial in a VA National Cemetery
VA Form VA40-10007 is the U.S. Department of Veterans Affairs Application for Pre-Need Determination of Eligibility for Burial in a VA National Cemetery. VA's page says to use it to find out in advance if you can be buried in a VA national cemetery or when you're applying on behalf of a Veteran or service member. The form says pre-need means before death, and not to complete it if the individual is already deceased.
The PDF has three pages. Section I covers the Veteran or servicemember and their military service, Section II the claimant, and Section III the certification and signature, followed by a page of instructions. The form says a pre-need determination of eligibility does not guarantee burial in a specific VA national cemetery, and that burial in a specific cemetery is scheduled at the time of need.
VA's page says you can apply online instead of filling out and sending the paper form. You can fill out the PDF free in your browser on this page and download the result without an account.
