Presidential Memorial Certificate Request Form
VA Form VA40-0247 is the U.S. Department of Veterans Affairs Presidential Memorial Certificate Request Form. The PDF itself prints the number as VA Form 40-0247. VA's form page says to use it to request a Presidential Memorial Certificate, an engraved paper certificate signed by the current president. It honors the memory of a deceased Veteran and expresses the country's grateful recognition of their service in the Armed Forces.
The form is one page in five sections: instructions, Veteran or Servicemember information, information about the person requesting the certificate, a certification and signature, and how to submit the form and documents. The footer says all versions of the form dated before May 2025 will not be accepted or processed.
VA's form page says a certificate can also be requested online instead of with the paper form. You can fill out the PDF free in your browser on this page and download the result without an account.

The requester certifies, to the best of their knowledge, that the decedent has never committed or been convicted of a serious crime, as the form describes, and signs item 13, which is required.

VA recommends attaching photocopies of readily available supporting documents, such as the most recent DD Form 214. If copies can't be found, the form says to apply anyway, as VA will attempt to obtain the records.
Complete Section II. The form says not to include nicknames, military rank or civilian titles, and marks the Social Security number as required.
Complete Section III. The form asks for a full name and complete mailing address to avoid delays in delivery, the number of certificates and the type of request.
Read the certification in Section IV and sign item 13, which the form marks as required.
Send the form and documents through Quick Submit, by mail or by fax as Section V lists, or request the certificate online through VA's page.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
