VA Form 27-2008: Application for United States Flag for Burial Purposes

VA Form 27-2008 is the U.S. Department of Veterans Affairs Application for United States Flag for Burial Purposes. It requests a United States flag that, as the form says, is issued on behalf of the Department of Veterans Affairs to honor the memory of one who has served our country. The instructions say to complete the form and submit it to any VA regional office or U.S. Post Office, and that when a burial is in a national, State, or military post cemetery, a burial flag will be provided by a funeral home.

The form is three pages. Page 1 holds information about the deceased Veteran, the flag recipient and the applicant, a certification and signature, and an acknowledgment of receipt signed by the person receiving the flag. Page 2 holds the instructions, and page 3 is a sheet on use of the flag that the issuing official detaches and presents to the recipient. The form says only one flag may be issued for each deceased Veteran.
