Veterans Mortgage Life Insurance Statement
VA Form 29-8636 is the U.S. Department of Veterans Affairs Veterans Mortgage Life Insurance Statement. VA's form page lists it under life insurance. The instructions describe Veterans Mortgage Life Insurance (VMLI) as designed to provide financial protection to cover an eligible veteran's outstanding home mortgage in the event of his or her death. They say the maximum amount of VMLI allowed is $200,000, and that the money is paid only to the mortgage holder.
The PDF has three pages. Page 1 describes the coverage, the mortgage, special provisions, the effective date, reporting changes and premiums. Page 2 explains how to complete each item. Page 3 is the statement: Part A to apply for the insurance, with a section for VA use, and Part B to decline it.

The form says to read the instructions before completing the statement, and that inaccurate information may result in not being insured for the full amount of the entitlement.
Complete Part A to apply for the insurance, or Part B to decline it.
Print or type the answers. The instructions say to insert "unknown" when information is not readily available, and that VA will secure it from other sources or write again.
