VA Form 29-0563: Veterans Mortgage Life Insurance Change of Address Statement

VA Form 29-0563 is the U.S. Department of Veterans Affairs Veterans Mortgage Life Insurance (VMLI) Change of Address Statement. It asks about the mortgaged property and the housing unit for which VMLI was granted: whether title to it was sold or otherwise disposed of, whether the Veteran occupies the residence, the final payoff of the mortgage loan and the last mortgage holder.

The form is one page. It also lets a Veteran who has purchased a home to use as a residence ask VA to send an application for VMLI on the new mortgage, and gives space to explain the reason for the change of address if the form does not apply. The Veteran signs and dates it. You can fill out the PDF free in your browser on this page and download the result without an account.
