Employer's Application to Provide Job Training
VA Form 22-8865 is the U.S. Department of Veterans Affairs Employer's Application to Provide Job Training, under Title 38 U.S. Code section 3677 or 3687. It is an employer form, not a Veteran or student application. It describes the trade, craft or occupation for which an establishment proposes to provide training to veterans and their eligible dependents. The form's privacy notice says VA cannot determine if a training program can meet the requirements set by law for VA approval unless the information is furnished.
The PDF has two pages. Page 1 holds Section I, employer identification, Section II, the description of the training program, Section III, the wage progression scale, and Section IV, the conditions to be met, followed by the signature, title and date. Page 2 holds the Privacy Act notice and the respondent burden statement.
