Designation of Certifying Official(s)
VA Form 22-8794 is the U.S. Department of Veterans Affairs Designation of Certifying Official(s). It is a school form, not a student or Veteran application. The form says it is used to provide the names and signatures of those individuals who are authorized to certify enrollment information to VA. VA's form page says to use it to tell VA about any changes related to school certifying officials (SCOs), and to include the names, titles and signatures of all certifying officials at the school or training facility, not just the information that has changed.
The PDF has three pages. Page 1 holds the instructions, the Section 305 training instructions, the school details and the primary certifying official. Pages 2 and 3 hold up to seven additional certifying officials, up to four read-only SCOs, remarks and the designating official's signature. VA's page says the form replaces the one submitted before.
VA's page says the form can also be submitted online. You can fill out the PDF free in your browser on this page and download the result without an account.

The complete name and address, including ZIP Code, of the school or training establishment, and its VA facility code if one is assigned.
Name, title, signature, telephone number and email, the date Section 305 requirements were met, and a yes or no answer on whether the person receives VA education benefits.
The same fields for up to seven more certifying officials, five on page 2 and two more on page 3.
Names of up to four read-only SCOs, numbered 1 to 4.
Remarks for extra space, the designating official's signature and date, printed name and title, email address and direct telephone number.
Page 3 states the penalty for a knowingly false statement of a material fact, the Privacy Act notice and a respondent burden of about 10 minutes.
The form says to include the names, titles and signatures of all certifying officials, not just the changed information, because each form supersedes the previous form.
New officials enter the date they completed new certifying official training and provide a copy of the certificate. Existing officials enter the annual training date. The form explains when to enter PENDING VA FACILITY CODE or EXEMPT instead.
Each official marks yes or no. The form says VA will not pay benefits for enrollment in a course certified by the individual taking the course.
The form says each certifying official signs on the same line as his or her name and title and gives a direct telephone number and email address.
The designating official signs and dates Items 4 and 5, then prints a name and title and gives an email address and direct telephone number in Items 6 to 8.
VA's page says the form can be submitted online instead of sending the paper form.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
VA's form page says: "You can submit your form online instead of sending us the paper form."
The form says newly designated certifying officials provide a copy of their training certificate when submitting this form.
Neither the PDF nor VA's form page prints a mailing address for the paper form.
The AUG 2024 edition supersedes the JUL 2021 edition, which the form says will not be used. It carries OMB Approved No. 2900-0262, which expires 08/31/2027.
