VA Form 22-10215: Statement of Assurance of Compliance with 85 Percent Enrollment Ratios

VA Form 22-10215 is the U.S. Department of Veterans Affairs Statement of Assurance of Compliance with 85 Percent Enrollment Ratios. It is a school form, not a student or Veteran application. VA's form page says school administrators use it to provide 85/15 calculations, and that only institutions of higher learning and non-college degree schools that charge tuition and fees should use it. The form cites 38 U.S.C. 3680A(d) and 38 CFR 21.4201.

The PDF has five pages: four pages of general information and block by block instructions, then the one page statement. The statement lists each approved program with its total enrollment, supported and non-supported students in full-time equivalents (FTE) and the supported student percentage, followed by a school official's certification and signature.

VA's page says the form can also be submitted online. You can fill out the PDF free in your browser on this page and download the result without an account.

What's on VA Form 22-10215

Blocks 1 to 4: institution and term

The institution name as listed on the WEAMS 22-1998 Report, the VA facility code with no spaces or hyphens, the term start date and the date the 85/15 calculation was performed.

Blocks 5A and 5B: programs and enrollment

Every approved program from the most recent WEAMS 22-1998 Report, even one with 0 students, and the total number of students enrolled in each program.

Block 5C: supported students enrolled

The number of supported students in the program. The instructions define a supported student, for example one receiving any amount of VA education benefits or an institutionally funded scholarship or grant. Under ten supported students, no more is needed for that line.

Blocks 5D and 5E: FTE counts

Supported and non-supported students in full-time equivalents. The instructions give the example of dividing the number of one-half time students by two and adding the result to the full-time students.

Blocks 5F and 5G: total FTE and percentage

Total enrolled FTE is 5D plus 5E, and the supported student percentage is 5D divided by 5F. The form's example is 15 divided by 40, which equals 37.5%.

Blocks 6 to 9: certification

A certification that the calculations are true and correct in content and policy, with the school official's printed name, title, signature and date. A checkbox notes attached continuation sheets, VA Form 22-10215a.
