Training Agreement for Apprenticeship and Other On-the-Job Training Programs
VA Form 22-8864 is the U.S. Department of Veterans Affairs Training Agreement for Apprenticeship and Other On-the-Job Training Programs, issued under Title 38 U.S.C. 3677 and 3687. It is completed by a training establishment, the employer, together with a trainee. The form's instructions to the establishment say to use it when providing on-the-job training (OJT) or apprenticeship training to veterans, their eligible dependents and members of the Selected Reserve who wish to use their VA Education Benefits while training with the facility.
The PDF has five pages: a page of instructions, then two copies of the agreement. Each copy has Part I on the training establishment, Part II on the trainee and Part III with the training agreement, wage progression, work processes and signatures. The back of each copy prints the Other On-the-Job Training Standards and the Apprenticeship Training Standards. The establishment keeps Copy 1 and gives Copy 2 to the VA trainee.

The trainee's name, address, Social Security number, VA file number, date of birth, job title or trade, credit for previous training or experience, the date training begins and the time remaining to be completed.
Specific qualifications for trainees, the ratio of trainees per instructor or apprentices to journey workers, a wage progression of up to ten periods and the present fully trained or journeyworker wage rate.
The operations or tasks to be learned, with a brief description and the hours of training for each, then any course curriculum or training outside the job and where it takes place.
The trainee and the establishment designee sign. The form says signing binds the parties to compliance with the Agreement and Training/Apprenticeship Standards.
The back of each copy covers hours and supervision, safety and health training, administrative procedures and Title VI of the Civil Rights Act of 1964, plus apprenticeship terms, probation, related school instruction and the certificate of completion.
The instructions say to read and complete the blanks under the apprenticeship training standards before completing the form, such as the term in years, the age range and the probationary period.
Enter the establishment details and the trainee details, including any credit for previous training or experience.
Complete the wage progression and list each task with its hours. The form says to continue on a separate sheet if more space is needed.
The trainee signs Item 6B and the establishment designee signs Item 7B. The form asks both to read the standards carefully before signing.
The instructions say to keep Copy 1 for your records and give Copy 2 to the VA trainee.
The standards make the establishment responsible for notifying the VA Regional Processing Office in writing of any interruption or termination of training, and for keeping a record of each trainee until 3 years after completion.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
Copy 1 stays with the establishment and Copy 2 goes to the VA trainee. The PDF does not print a VA mailing address.
