Monthly Certification of Flight Training
VA Form 22-6553c is the U.S. Department of Veterans Affairs Monthly Certification of Flight Training. It is a flight school form, not a student application. Its instructions are headed Instructions for Flight School Operators, and the certification on page 1 is signed by the school certifying official. It reports the flight instruction a student received during a reporting period, what it cost the student and the hours given to date against the maximum hours approved for the course.
The form is two pages. Page 1 holds Items 1 to 15: the student, the reporting period, the instruction furnished, costs, a summary of hours, remarks and the certification. Page 2 holds the instructions with a worked example for Item 7, the Privacy Act notice and the respondent burden. You can fill out the PDF free in your browser on this page and download the result without an account.

The student's name and file number, the reporting period's beginning and ending dates, a changed address, the date training was completed or terminated and the name of the current course.
Each type of instruction given for the current course during the period, with horsepower, hours, rate and amount.
The cost to the student for instruction this period excluding taxes, any state and local sales taxes, the total cost this period and the total cost for the course through the end of the period.
Maximum approved hours and hours given to date for dual, dual simulator (not included in dual), solo, pre-flight and post flight, ground and other instruction.
Any substitution, flight test or variance from the approved course.
The certifying official's printed name, signature and date, then the name and address of the flight school and its facility code.
The instructions say to complete the form after the last day of the month, unless the student's training was interrupted or completed earlier.
Show the reporting period dates in Item 3 and the course name in Item 6. If the student completes or interrupts the course, enter the date in Item 5 and the reason in Item 11.
Item 7 lists each type separately. Flight time follows Part 1.1 of FAR 1, from the moment power is applied for the purpose of flight until the plane lands and taxis to a final rest.
Item 8 is the charges for the period and Item 9 the total since the course began. Item 10 shows maximum approved and cumulative hours. The form says instruction may not exceed the approved maximum hours.
Use Item 11 for any substitution, flight test, variance or change in enrollment and training status, such as an aircraft not approved for the course.
The school certifying official signs Item 13A in ink and dates Item 13B. The form goes to the appropriate Regional Office as early as possible after the end of the month.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
