Education Benefit Entitlement Restoration Request Due to School Closure, Program Suspension or Withdrawal (VA Form 22-0989)
VA Form 22-0989 is the U.S. Department of Veterans Affairs Education Benefit Entitlement Restoration Request Due to School Closure, Program Suspension or Withdrawal. VA's form page says to use it to request restoration of your education benefit entitlement if you couldn't complete your educational program because your school closed permanently or lost its VA approval for the program you were enrolled in at that school.
The instructions also mention a school that, due to COVID-19 and between March 1, 2020 and June 1, 2022, closed temporarily or suspended a program of study. They list the benefits restoration can be granted for: the Post-9/11 GI Bill (Chapter 33), DEA (Chapter 35), the Montgomery GI Bill (Chapter 30), the Montgomery GI Bill Selected Reserve (Chapter 1606) and VEAP (Chapter 32).
The PDF is two pages: an instructions page, then the request in three parts. VA's form page says you can also apply online. You can fill out the PDF free in your browser on this page and download the result without an account.

Items 1 to 5: name, VA file number or Social Security number if the file number is unknown, current mailing address, home and mobile telephone numbers and email address.

Whether you are enrolled at a new school and its name and program, a teach-out agreement, whether it granted 12 or more credit hours, credit transfers from a non-college degree school, your last date of attendance and remarks.
A signed acknowledgment that you transferred fewer than 12 credits from the closed or disapproved facility, and that the restored entitlement will be rescinded or revoked if 12 or more credits are transferred later.

VA's form page lists a school that closed permanently as one reason to request restoration of education benefit entitlement.
It also lists a school that lost its VA approval for the program you were enrolled in at that school.
The instructions include a school that, due to COVID-19 and between March 1, 2020 and June 1, 2022, closed temporarily or suspended your program of study.
