Application for Family Member to Use Transferred Benefits
VA Form 22-1990e is the U.S. Department of Veterans Affairs Application for Family Member to Use Transferred Benefits. The form says it is used to apply for Transfer of Entitlement (TOE) to basic educational assistance under chapters 30 and 33 of title 38, U.S. Code and chapter 1606 of title 10, U.S. Code. VA's form page says to use it if you're the dependent of a Veteran or service member who has already transferred Post-9/11 GI Bill benefits to you.
The PDF has four pages: two pages of instructions and regional office addresses, then the application in Parts I to VI. The form says not to use it for benefits based on your own military service, and points to VA Form 22-1990 for that.
VA also accepts this application online. You can fill out the PDF free in your browser on this page and download the result without an account.

Social Security number, sex, date of birth, name, address, mobile and home phone, an email address if applicable, direct deposit details, relationship to the service member and high school diploma or equivalency.
Item 9A selects the benefit transferred: chapter 33 Post-9/11 GI Bill, chapter 30 MGIB or chapter 1606 MGIB-SR. Item 9B lists types of training, then the school and your objective if known.

Any FAA flight certificates, education after high school with dates, hours, credentials and major field, and up to two jobs since high school.

Whether active duty applicants expect money from the Armed Forces or Public Health Service, and whether federal civilian employees expect money from their agency, for the same training.

The service member's Social Security number, branch of service, name and address.

A certification that the statements are true and correct, a penalty notice, and the signature of the applicant or parent, guardian or custodian with the date signed.
The form says the instruction numbers match the item numbers on the application, and items not mentioned are self-explanatory.
Give your details, the benefit transferred to you in Item 9A, your type of training in Item 9B and the service member's information in Part V.
Give the routing number, account type and account number in Item 7, and attach a voided personal check or deposit slip that matches.
Check applicant or parent, guardian or custodian, sign without printing and date it. If on active duty, the certification also says you consulted an Education Service Officer.
Mail it to the VA Regional Processing Office for your school's region, or your home address region if no school is chosen. VA's page says you can submit online instead.
The form says to tell the veterans certifying official at your school that you applied and ask them to submit your enrollment information on VA Form 22-1999, Enrollment Certification.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
