Request for Reimbursement of Licensing or Certification Test Fees (VA Form 22-0803)
VA Form 22-0803 is the U.S. Department of Veterans Affairs Request for Reimbursement of Licensing or Certification Test Fees. VA's form page says to use it when you want to use your VA education benefits to get paid back for licensing and certification tests. The form says to use one application for one test only.
The form is two pages. Page 1 asks which education benefit the request falls under, then has Part I identification, Part II VA education information, Part III test information and Part IV certification and signature. Page 2 has the item instructions, help contacts and the two Regional Processing Office addresses. The form says applicants who have not already done so apply separately and are found eligible for the education benefit first. VA's page says you can apply online instead of sending the paper form. You can fill out the PDF free in your browser on this page and download the result without an account.

Check boxes at the top select the education benefit under which you are requesting test fee reimbursement.

Items 1 to 5: your name, mailing address, VA file number, email address and mobile or home telephone number.

Item 6 asks whether you have previously applied for VA education benefits.

The name of the test, the name and mailing address of the organization issuing the license or certification, the date taken and results, the total cost including mandatory fees, and remarks.

A certification that the information is true and correct to the best of your knowledge and belief, your signature in Item 12 and the date in Item 13.
Check the benefit at the top. The form says applicants who have not already done so apply separately for that benefit, online at https://www.va.gov/education/how-to-apply/.
Enter your identification details. The instructions say your VA file number is the number on your VA benefit payments and mail, and is generally your Social Security number.
Give the complete test name and the organization issuing the license or certificate, which the instructions say is not necessarily the organization that administered the test.
The form asks for a copy of the test results and a payment receipt. Without results, the instructions say to attach a copy of the license or certification and the payment receipt.
The instructions say VA reimburses required test fees, such as registration, specialized test and proctoring fees, and has no authority to reimburse optional costs such as pre-tests or fast scores.
Sign and date Items 12 and 13, then upload it through QuickSubmit on AccessVA, mail it to your Regional Processing Office, or apply online.
