Contractor Production Report
VA Form VA10101 is the U.S. Department of Veterans Affairs Contractor Production Report. It records one day of work on a construction contract: the contract number, the title and location of the construction contract, the contractor and superintendent, the weather, the work performed, job safety, equipment and material received and the equipment on site.
It is a contractor form, not a benefits form. The signature line is for the contractor or superintendent, with the date the report was prepared. The form asks for additional sheets if necessary.
The PDF is one page. It carries OMB No. 2900-0208 and an estimated burden of 24 minutes per respondent. You can fill out the PDF free in your browser on this page and download the result without an account.

List each activity with its schedule activity number, employer, trade and hours, and add the total and cumulative work hours. Attach additional sheets if needed.
The form asks for meeting minutes if a safety meeting was held, a completed OSHA report if there was a lost time accident, and an inspection statement or checklist for high risk work.
Note the equipment and material received with its schedule activity and submittal numbers, and the construction equipment used with owner, make and model.
The contractor or superintendent signs and enters the date the report was prepared.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The form prints no mailing address or submission method.
The form gives [email protected] for comments on the burden estimate only, and says not to send the completed VA Form 10101 to that address.

The Work Performed Today table lists each employer with the number of workers, their trade and the hours worked.
The construction and plant equipment table names the owner of each piece of equipment used that day, with its make and model.
