Application to Make Service Credit Payment - FERS
Standard Form 3108 (SF 3108), Application to Make Service Credit Payment, Federal Employees Retirement System, is a U.S. Office of Personnel Management (OPM) form, Revised June 2013, OMB No. 3206-0134. GSA hosts a copy in its forms library.
The form says it is for people covered by FERS who want to pay for civilian service so that they can receive retirement credit for it, including service that will be credited under Civil Service Retirement System (CSRS) rules. Current FERS employees also use it with SF 3108A, Application to Pay Military Deposit, for military service performed after December 31, 1956. For employees under CSRS, the form points to Standard Form 2803.
The package holds Part A for the applicant, Part B for the employing agency, an information sheet with FERS service credit rules and rates, and SF 3108A. The form says payment is optional. OPM estimates an average of 30 minutes per response. You can fill it out free in your browser on this page and download the result without an account.

Name, other names, birthdate, address, agency and location of employment, Social Security Number, position title, and any earlier FERS or CSRS application with its type and claim number.
Each period of civilian service with whether deductions were not withheld or were refunded, a check mark beside each period to pay for, current FERS deductions or separation date, and signature.

Any election to transfer to FERS, verified civilian service and its retirement system, civilian service not under FERS or CSRS with salary details, comments and the agency certification.

A sheet to detach and keep, with general FERS rules, payment rates for civilian, Peace Corps and VISTA service, interest rates, and how to apply and make payment.
The employee lists each branch and period of military service, the agency marks the retirement system rules, USERRA and the interest accrual date, and the employee signs an acknowledgment.
The form says to read the attached information carefully, type or print in ink, and complete Part A in full to avoid a delay in processing.
List all periods of Federal civilian service in chronological order, and put a check mark next to each period you want to pay for. A box left blank means that period is not included.
If you are a Federal employee, the form says to send the completed application to your department or agency because they certify it.
Complete item 1 and sign item 2 of SF 3108A, and include a DD 214 or equivalent record and documentation of military basic pay. The form says the agency computes the deposit.
The form says OPM sends instructions for making payment, together with a bill for the amount due, once it processes the application. Installment payments of at least $50.00 are acceptable.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
Send the completed application to your department or agency. The form says not to file if you plan to retire within six months, since OPM gives an opportunity to pay when it computes the annuity.
The form says to send it directly to Office of Personnel Management, Retirement Operations Center, P.O. Box 45, Boyers, PA 16017-0045.
The form gives OPM at 1-888-767-6738 (TTY: 1-855-887-4957), Monday through Friday from 7:30 AM to 7:45 PM Eastern Time, and points current employees to their agency or human resources office.
Payments can be made by direct debit from a checking or savings account with form RI 16-28, Authorization for Direct Payments, which the form says is at www.opm.gov/forms.
Title 5, U.S. Code, Chapter 84 authorizes the collection. The form says failure to furnish the data will delay or prevent action. OPM estimates an average of 30 minutes per response.
Employees covered by FERS who want to pay for civilian service complete Part A. The form says a current Federal employee has the employing agency complete Part B.
The form says someone who left FERS covered employment with at least 5 years of paid creditable civilian service may also use it to pay for additional unpaid civilian service.
The form says only current FERS covered employees may apply to pay for military service performed after December 31, 1956, using SF 3108A with SF 3108.
The agency completes Part B from official records and the Agency Use Only columns of SF 3108A, and an authorized official signs each certification.

It is printed as a U.S. Office of Personnel Management form, Revised June 2013, OMB No. 3206-0134. GSA hosts a copy in its forms library.
The form says payment is optional and you do not have to pay if you do not want to do so. It then describes how unpaid periods are treated in the annuity computation and eligibility.
No. The form says employees under the Civil Service Retirement System use Standard Form 2803, Application to Make Deposit or Redeposit, to apply to pay for service.
The form says to complete SF 3108 and SF 3108A, include a DD 214 or equivalent record and military basic pay documentation, and give them to your employing agency, which computes the deposit.
The form says your agency can tell you which military pay records are acceptable, and that estimated earnings can be requested with RI 20-97. It says tax and Social Security earnings statements may not be used.
The form says installment payments of at least $50.00 are acceptable, and that installments cost additional interest because interest is charged on the unpaid balance.
