Request for Recovery of Debt Due the United States
Standard Form 2805 (SF 2805), Request for Recovery of Debt Due the United States, is a U.S. Office of Personnel Management (OPM) form, Revised December 2005, under the CSRS/FERS Handbook for Personnel and Payroll Offices. GSA hosts a copy in its forms library. The footer says it is authorized for local reproduction and that previous editions are not usable.
The form is completed by a creditor agency, not by the debtor. An agency official certifies that an annuitant or former employee covered by CSRS or FERS owes the United States a debt in the amount certified, and that the procedures in 31 CFR 901, et. seq., and in 5 CFR 831.1801, et. seq., or 5 CFR 845 have been followed. OPM then completes the Report of Action on Request for Recovery at the bottom.
