Statement and Acknowledgment
Standard Form 1413 (SF 1413), Statement and Acknowledgment, is a two-sided record of a subcontract awarded under a federal prime contract. The footer says it is prescribed by GSA under FAR (48 CFR) 53.222(e), and GSA hosts it in its forms library. The edition printed on the form is STANDARD FORM 1413 (REV. 10/2023), with OMB Control Number 9000-0066, and the footer says the previous edition is not usable.
Part I is the Statement of Prime Contractor: the prime contract number, the subcontract number and award date, both firms' names and addresses, whether the prime contract contains the Contract Work Hours and Safety Standards Act overtime compensation clause, and a description of the subcontractor's work. Part II is the Acknowledgment of Subcontractor, where the subcontractor acknowledges that listed labor standards clauses of the prime contract are included in the subcontract.
The Paperwork Reduction Act statement estimates .05 hours to read the instructions, gather the facts and answer the questions. You can fill it out free in your browser on this page and download the result without an account.
