Labor Standards Interview
Standard Form 1445 (SF 1445), Labor Standards Interview, is a one-page record of an interview with an employee working under a contract. The footer says it is prescribed by GSA under FAR (48 CFR) 53.222(g), and GSA hosts it in its forms library. The edition printed on the form is STANDARD FORM 1445 (REV. 12-96), and the footer says the previous edition is not usable.
The form records the contract number, the prime contractor, the employer and the supervisor, then the employee's name, address, work classification and wage rate. It asks the employee yes or no questions on working over 8 hours per day and over 40 hours per week, overtime pay and cash payments for fringe benefits, plus deductions, hours worked, tools used and start dates. An interviewer and a payroll checker then add their own entries and signatures.
The PDF prints no instructions, OMB number or filing address. You can fill it out free in your browser on this page and download the result without an account.
