Labor Standards Investigation Summary Sheet
Standard Form 1446 (SF 1446), Labor Standards Investigation Summary Sheet, is a one-page summary of a labor standards investigation of a contract. The footer says it is prescribed by GSA under FAR (48 CFR) 53.222(h), and GSA hosts it in its forms library. The edition printed on the form is STANDARD FORM 1446 (REV. 4/2013), and the footer says the previous edition is usable.
The sheet records the reporting office, the contract number, amount, date and type, the contractor and any other employer, the project and the work, the basis for the investigation and the wage determination. It then summarizes the nature and extent of any violation, including underpayments under the Construction Wage Rate Requirements Statute and the Contract Work Hours and Safety Standards Statute, and the corrective actions taken, such as restitution and payments withheld.
The PDF prints no instructions, OMB number or filing address. You can fill it out free in your browser on this page and download the result without an account.

The first block on the sheet is the Reporting Office, the office the summary comes from.
The Prepared By block at the bottom gives the date, title and signature of the person who prepared the sheet.
The contractor's name and address, including ZIP code, is recorded on the sheet. The contractor does not sign it.
A separate block records the employer's name and address if other than the prime contractor, and the sheet counts the employees involved.
The PDF prints no instructions. Only the preparer signs; the other blocks identify the parties and the contract.
