Request for Official Personnel Folder (Separated Employee)
Standard Form 127 (SF 127), Request for Official Personnel Folder (Separated Employee), is a National Archives and Records Administration (NARA) form. Its footer prints STANDARD FORM 127 (02-11) and Prescribed by NARA, 36 CFR 1233.18(c)(1). GSA hosts a copy in its forms library.
The form is addressed to the National Personnel Records Center (Civilian Personnel Records) and asks it for the official personnel folder of a separated federal employee. GSA's page says use of the SF 127 is restricted to authorized representatives of Federal personnel offices. Sections I and III are completed by the requesting personnel office, and Section II is for the records center's reply. The form says one form is required for each folder requested, and that requests may be faxed or mailed to the National Personnel Records Center.
