Records Transmittal and Receipt
Standard Form 135 (SF 135), Records Transmittal and Receipt, is a records management form. Its footer says it is prescribed by NARA under 36 CFR 1228.152, and GSA hosts a copy in its forms library. The form says to complete and send the original and one copy to the appropriate Federal Records Center for approval prior to shipment of records.
The transferring agency fills in the records center address, its own transfer authorization, a liaison contact and the office retiring the records. The Records Data table then lists each series of records: an accession number, volume in cubic feet, box numbers, a series description, a restriction code, the disposal authority and the disposal date. The records center signs the receipt and completes the last four columns.
You can fill it out free in your browser on this page and download the result without an account.

A separate accession number is required for each series. It has three parts: the NARA record group number, the last two digits of the current fiscal year, and a four digit sequential number obtained in advance from the records center.
Enter the volume in cubic feet and the inclusive box numbers, and describe the records in enough detail for the records center to check the disposal schedule, with their inclusive dates.
The form says to number each carton sequentially, such as 1 of 25, 2 of 25, starting each new series at carton number 1, and to mark each container with its accession number prior to shipment.
Enter a restriction code in column (g), the agency schedule and item number authorizing disposal in column (h), and the month and year the records may be destroyed in column (i).
