SF115
Request for Records Disposition Authority
Standard Form 115 (SF 115), Request for Records Disposition Authority, is addressed to the National Archives and Records Administration (NARA). The footer says it is prescribed by NARA under 36 CFR 1228, and GSA hosts a copy in its forms library. GSA's page lists 36 CFR 1228 as its authority.
The instructions on the back say to use SF 115 to obtain authority for the disposition of records. The submitting agency describes each records series and its proposed disposition, and an authorized agency official signs a certification. NARA fills in the job number, the action taken on each item and a notification to the agency with a line for the Archivist of the United States.
The form has 10 numbered entries on the front and instructions on the back. You can fill it out free in your browser on this page and download the result without an account.

The agency or establishment, major and minor subdivisions, and the name and telephone number of the person to confer with.
The signer certifies the records proposed for disposal on the attached pages are not now needed, or will not be needed after the retention periods, and checks whether GAO concurrence is not required, is attached or has been requested.
Item numbers in sequence and a description of each records series with its proposed disposition.
Column 9 holds the GRS or superseded job citation. Column 10, Action Taken, is for NARA use only.
Name the agency, the subdivisions that create or maintain the records, and the person to contact with a telephone number.
Entry 7 numbers the records in sequence beginning with 1. The instructions allow 1a, 1b, 1b(1) and so on for subdivisions, and say agency file numbers go in entry 8, not entry 7.
The instructions ask for headings by office of origin, separate items for nontextual records such as photographs or maps, and clear disposition instructions with file breaks and retention periods.
Enter the previous NARA job and item numbers, General Records Schedule items and agency directive items, if applicable. The instructions say to leave it blank only for records scheduled for the first time.
The authorized agency official signs and dates entry 6 and checks the GAO concurrence box that applies.
The instructions say to submit two signed copies to NARA (NIR) and keep one copy as a suspense copy.

The form is addressed to National Archives and Records Administration (NIR), Washington, DC 20408. The instructions say to submit two signed copies and retain one copy as your suspense copy.
NARA later returns one copy as notification of the items approved for disposal or archival (permanent) retention, and that copy also shows any items withdrawn or disapproved.
The SF 115 may be accompanied by Standard Form 115A, Continuation Sheet, by schedule items on blank stationery formatted like the SF 115A, or by pages matching the agency's published records disposition schedule.
The form says GAO's written approval either accompanies each SF 115 needing Comptroller General concurrence or is requested before the SF 115 is submitted to NARA.
The instructions say unsigned SF 115s are returned, an SF 115 that does not meet 36 CFR 1228 is returned for corrective action, and one missing column 9 information is returned without action.
Entry 1 names the Executive Branch department or independent agency, Legislative Branch agency, or the Administrative Office of the U.S. Courts for the Judicial Branch that is submitting the request.
Entries 2 and 3 show the major and minor organizational subdivisions that create or maintain the records. If more than one office does, the instructions say to name them in entry 8.
Entry 6 is signed and dated by the agency official authorized to certify that the retention periods are adequate and that GAO requirements have been met. Unsigned SF 115s are returned to the agency.

For records converted to electronic form, the instructions say to schedule both the original records and the electronic media, unless covered by the General Records Schedules. For permanent or unscheduled records converted to microform, both copies are approved on an SF 115 with the certifications required by 36 CFR 1230.
Column 10, Action Taken, is for NARA use only and the instructions say to leave it blank. NARA also fills in the job number and date received at the top.
