Standard Form 152 (SF 152): Request for Clearance or Cancellation of an SF or OF

Standard Form 152 (SF 152), Request for Clearance or Cancellation of a Standard or Optional Form, is a forms management request. Its footer says it is prescribed by GSA under FMR (41 CFR) 102-194, and GSA hosts it in its forms library. This edition is Rev. 11/2000, and the form says the previous edition is not usable.

A requesting department or agency uses it for a new, revised or cancelled Standard Form (SF) or Optional Form (OF), or another action it specifies. The instructions say to submit a separate set of clearance documents for each form and to forward them, with the listed items, to General Services Administration (Forms-XR), 1800 F Street, NW, Washington, DC20405. An approval block is marked GSA only.

The back covers stock evaluation and disposition, procurement and stocking, printing specifications, proofs and samples. You can fill it out free in your browser on this page and download the result without an account.
