Optional Form 8 (OF 8): Position Description

Optional Form 8 (OF 8), Position Description, is a U.S. Office of Personnel Management (OPM) form. Its footer reads OF 8 (Rev. 1-85), U.S. Office of Personnel Management, FPM Chapter 295, and says the previous edition is usable. GSA hosts a copy in its forms library, and GSA's page lists the authority as Federal Personnel Manual (FPM) Chapter 295.

The form records a federal position: its agency position number, reason for submission, location, Fair Labor Standards Act status, position status, sensitivity and classification or job grading. The description of major duties and responsibilities is typed separately and attached under item 25. The immediate supervisor certifies the description, and the official who makes the classification decision certifies the grade.

The back of the form carries instructions for each of the 25 items. It says agencies must complete the items marked by an asterisk and may determine what other items are used. You can fill it out free in your browser on this page and download the result without an account.

How to complete OF 8

  1. 1

Read the instructions on the back

The back explains each item and marks the required ones with an asterisk. It points to FPM chapters for terms such as competitive level code and FLSA status.

  2. 2

    Show the reason for submission

Check Redescription, New, Reestablishment or Other in item 2. The instructions say the Explanation section shows the reason if Other is checked, and any positions replaced.

  3. 3

    Enter the classification action

In item 15, the official title comes from the applicable classification or job grading standard. For scientific and engineering occupations, the instructions say to add the two-digit functional classification code in parentheses, e.g., GS-1310(14).

  4. 4

    Attach the duties

Item 25 says to type the description on plain bond paper and attach it to the form, with the agency position number shown on the attachment.

  5. 5

    Certify the description

If the position is occupied, the employee reads the description (item 19). The immediate supervisor signs item 20 and the classifying official signs item 21.

  6. 6

    Review it periodically

The instructions say agencies are encouraged to review each established position periodically, and item 23 may be used for that review. Supervisor and classifier initials there represent recertifications of items 20 and 21.
