OF720
Senior Executive Performance Plan
Optional Form 720 (OF 720), Senior Executive Performance Plan, is a performance plan and appraisal record for a senior executive. The PDF names no issuing agency. GSA hosts it in its forms library, and both the form footer and GSA's page show the 10/2017 edition.
The form records consultation on the plan, a progress review, the initial and annual summary ratings, and any higher level review and Performance Review Board recommendation. It rates five critical elements: Leading Change, Leading People, Business Acumen, Building Coalitions and Results Driven, with weights that total 100 points.
OF 720 runs to six pages and ends with the rating official's summary narrative, an optional executive's accomplishment narrative and a block for agency use. You can fill it out free in your browser on this page and download the result without an account.

The executive's name, title, organization and appraisal period, the rating official's name, CA, NC and LT/LE check boxes, and dated signatures for the consultation and the progress review.

The initial summary rating from Level 5 Outstanding to Level 1 Unsatisfactory, the higher level review, the Performance Review Board recommendation and the annual summary rating, each with signatures and dates.

A table of the five critical elements with initial and final element ratings, weights and scores. The summary level ranges are 475 to 500 for Level 5, 400 to 474 for Level 4, 300 to 399 for Level 3 and 200 to 299 for Level 2, and any element rated Level 1 makes the summary Level 1.

A written standard for each of the five levels and the element rating points: Level 5 = 5, Level 4 = 4, Level 3 = 3, Level 2 = 2 and Level 1 = 0.
Each element prints a mandatory performance requirement, space for agency-specific requirements, an optional rating official narrative and a rating from Level 5 to Level 1. Results Driven has five performance requirement boxes, each with a strategic alignment box.
The rating official's summary narrative (Mandatory), the executive's accomplishment narrative (Optional) and a block for agency use.
Enter the executive, title, organization and appraisal period. The executive and the rating official sign and date Part 1, where the executive states the plan was reviewed and the executive was consulted on its development.
Enter a weight for each critical element. The form prints a minimum weight of 5 points for elements 1 to 4 and 20 points for Results Driven, with a total of 100 points.
The form says this element must have at least 1 performance requirement with measurable results and quality indicators describing performance at Level 3, and asks for the strategic alignment of each requirement.
The executive and the rating official sign and date Part 2. A reviewing official may also sign.
The rating official marks a level for each critical element. Part 4 records each element's rating, weight and score, and the total is read against the summary level ranges printed beside it.
After the initial summary rating, the executive may request a higher level review. The PRB Chair signs the board's recommendation and the appointing authority signs the annual summary rating.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The PDF gives no filing address and names no office that keeps the plan. Its last block, Part 8, is for agency use.
Part 5 says examples for the top three performance levels can be found in the system description, which is not part of the PDF.
The reviewing official's signatures, the narrative under each element and the executive's accomplishment narrative are marked Optional, and the Higher Level Review section is marked if applicable.
Each page footer reads OPTIONAL FORM 720 10/2017, and GSA's page lists 10/2017 as the current revision date.
