OF294
Emergency Equipment Rental and Service Agreement
Optional Form 294 (OF 294), Emergency Equipment Rental and Service Agreement, is a one-page agreement between a procurement agency and a contractor for equipment or services on an incident. Its footer prints OPTIONAL FORM 294 (REV. 1/2026) and USDA/USDI. GSA hosts a copy in its forms library.
The agreement records the procurement agency, the contractor and its Unique Entity Identifier (UEI), the agreement number and effective dates, the incident, the point of hire and ordering dispatch center, each item or resource with its rates, and special provisions. The contractor or an authorized agent signs, and so does the contracting officer. The form says the General Clauses to the agreement are attached; they are not part of this one-page PDF.
You can fill it out free in your browser on this page and download the result without an account.
