Property Loss or Damage Report - Fire Suppression
Optional Form 289 (OF 289), Property Loss or Damage Report, Fire Suppression, is a one-page report on property lost or damaged in fire suppression. Its footer prints OPTIONAL FORM 289 (9-81) and USDA/USDI, with NSN 7540-01-124-7634. GSA hosts a copy in its forms library.
The form identifies the crew, the fire and the person the property was issued to, lists up to three items with quantities, and then gives space for three reports: the employee's account of the loss or damage, a witness report, and comments from the Fire Boss or Property Control Officer. Each report has its own signature and date.
