Bill of Lading Accountability Record
Optional Form 1121 (OF 1121), Bill of Lading Accountability Record, is a two-sided record for a range of bill of lading numbers. Its footer prints OPTIONAL FORM 1121 (12-79) and says it is authorized for local reproduction. The form names no issuing agency, and GSA hosts it in its forms library.
Each side has lines for the range of bill of lading numbers and for who they were issued to and reissued to, with dates, then a table with B/L No., Furnished To and Date Memo Received columns. The B/L No. column has a preprinted digit on each row, with 24 rows on each side. It prints no instructions, no signature line and no address.
You can fill it out free in your browser on this page and download the result without an account.
