Standard Form 1200 (SF 1200): Government Bill of Lading Correction Notice

Standard Form 1200 (SF 1200), Government Bill of Lading Correction Notice, is a one-page notice that shows how a Government Bill of Lading (GBL) reads now and how it should read. The footer says it is prescribed by GSA, citing FPMR (41 CFR) 101-41.3, and GSA hosts a copy in its forms library. GSA's page lists no authority or regulation.

The form identifies the GBL by number, issue date, total weight, origin, destination, route and issuing office, as shown on the GBL. It is directed to a carrier or activity in block 8, and asks for the wording before and after the correction, the authority for the change, remarks and who gets an information copy. The initiating official signs block 16, and block 17 is for the carrier representative's signature in the cases the form names.

The footer prints NSN 7540-01-140-5524. You can fill it out free in your browser on this page and download the result without an account.
