GSA Form 5029: FLSA Exemption Determination Checklist, Learned Professional Exemption

GSA Form 5029, FLSA Exemption Determination Checklist, Learned Professional Exemption, is a two-sided General Services Administration (GSA) form printed as GSA 5029 11/2017. GSA hosts it in its forms library. The form prints no OMB number, and GSA's page lists no authority or regulation.

The checklist works through whether the employees in a position meet the definition of an FLSA-exempt learned professional. Its questions have YES and NO columns, and the go-to boxes send a NO answer straight to the final question. It cites 5 CFR 551.203, 5 CFR 551.208 and 5 CFR 551.104.

The form says the employee should be marked as exempt only if every question is answered YES, and that a NO to any question means the employee does not meet the definition for the learned professional exemption. You can fill it out free in your browser on this page and download the result without an account.
