Customer Supply Center Order
GSA Form 3542, Customer Supply Center Order, is a General Services Administration form, GSA FORM 3542 (REV.3-92). GSA hosts it in its forms library; GSA's page lists no authority or regulation.
It is an order for up to 17 items from a Customer Supply Center (CSC). Each line takes the last 7 digits of the NSN, a short description, the unit of issue, the quantity, the unit price and an estimated extended price, as found in the CSC Catalog. The order is signed by the person submitting it and by an approving official, and carries the customer address, activity address code, agency access code and fax number.
The instructions on the back say to mail or fax the signed, original order form to the Customer Supply Center shown on the front of the form, and not to use this form to confirm telephone orders. You can fill it out free in your browser on this page and download the result without an account.
