GSA Form 220: Inspection Report on Work Under Contract

GSA Form 220, Inspection Report on Work Under Contract, is a General Services Administration form, Rev. 9/2025. GSA hosts it in its forms library, and GSA's page lists the PBS Project Management Guide as its authority. GSA's listing titles it Inspection Report for Work Under Contract.

The instructions on the back say GSA 220 is used for documenting contract compliance by the Contracting Officer's Representative (COR) on all construction contracts awarded under new construction and repair and alteration programs. The form says it documents the required separation of duties between the ordering and inspecting functions, and that the COR is the GSA responsible party for accepting the work for payment purposes.

The front has 26 numbered blocks covering the contract, prices, completion dates, the firms and inspector involved, the inspection and the COR's approval. The back holds the instructions, the final inspection procedure, reinspection costs and a definition of substantial completion. You can fill it out free in your browser on this page and download the result without an account.

Final report to the Regional Office

The instructions say to forward to the Regional Office the final report, three copies of the list of defects and omissions given to the contractor, and a SEPARATE supplementary statement in duplicate with an estimated cost of completing each item.
