Construction Progress Report
GSA Form 184, Construction Progress Report, is a two-page General Services Administration (GSA) form, printed as GSA 184 (REV. 9/2025). GSA hosts it in its forms library, and GSA's page lists GSAM 552.232-5 and FAR 52.232-5 as its authority or regulation.
The instructions say GSA 184 shall be prepared for each progress payment prepared in accordance with the Federal Acquisition Regulation, FAR 52.232-5. The front records the contract, the work status, completion dates, percent completed and a 15-line progress payment summary that ends with the amount of payment this report. Its lines draw on GSA 184A for the work breakdown and options and GSA 184B for modifications and change orders.
The back has space to report circumstances that adversely affected progress, followed by instructions and definitions. The form has blocks for Prepared By, Work Accepted By (Project Manager) and two Approved By signers. You can fill it out free in your browser on this page and download the result without an account.

Page of pages, report number or invoice number, date, location, title, contract number, task order number and contractor.

Completion dates (estimated substantial, actual substantial, initial contract, revised contract) and percent completed through this month and normal to date.
Average work force number employed, and yes or no for satisfactory progress, material delivery, construction equipment and shop drawing or sample submissions.
Initial contract amount (design phase or pre-construction services, bonds, construction services), contract modifications and change orders from GSA 184B, options exercised, and the total contract amount to date.
Value of work completed, work under modifications, material on site and in storage, less retainage and withholdings, bond premiums, less previous payments, and the amount of payment this report.
Space to report circumstances such as strikes, weather or delays by the Government, explain any No answers and describe the work with its CLINs and Sub-CLINs, then the instructions and definitions.
The instructions say GSA 184 shall be prepared for each progress payment prepared in accordance with FAR 52.232-5.
The form figures percent completed through this month as the sum of Line 5, Line 6 and Line 12 divided by Line 4. The instructions call it a financial completion.
Mark yes or no for progress, material delivery, construction equipment and submissions, and explain any No answer on the back.
Each total line prints its formula: Line 4 is Lines 1, 2 and 3; Line 8 is Lines 5, 6 and 7; Line 10 is Line 8 minus Line 9; Line 13 is Line 10 plus Line 12; Line 15 is Line 13 minus Line 14.
Item 3 is for phased construction, design/build or Construction-Manager-as-Constructor (CMc) where the option has its own Work Breakdown schedule of values on a separate 184A. Otherwise the option modification goes on the 184B.
Fill in Prepared By, Work Accepted By (Project Manager) and the two Approved By blocks, each with a date.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The PDF gives no filing address and names no office that receives the report.
The form defines it as the Contractor having completed the Work so the Government may enjoy its intended use without impairment, and says the Work is never Substantially Complete if required fire and life safety systems are not tested and accepted by the Authority Having Jurisdiction.
The form defines retainage as before SC up to 10%, and after SC whatever is required to protect the government's interest. Item 7 covers materials that have not been installed but accepted by the Government.
The PDF prints no OMB number, time estimate or Privacy Act statement.
GSA's page lists form number GSA184, a current revision date of 09/2025 and GSAM 552.232-5 and FAR 52.232-5 as the authority or regulation.
