Cover Sheet for Reaffirmation Agreement
Form B 427, Cover Sheet for Reaffirmation Agreement, is the Official Bankruptcy Form (Official Form 427) used in a case in the United States Bankruptcy Court. The U.S. Courts list it under Bankruptcy Forms. The current version is dated 12/15, and the U.S. Courts page lists it as effective on December 1, 2015. The page says the form is derived from Official Form 27, and that Official Bankruptcy Forms are approved by the Judicial Conference and must be used under Bankruptcy Rule 9009.
The form says anyone who is a party to a reaffirmation agreement may fill it out and file it, attached to the reaffirmation agreement, within the time set under Bankruptcy Rule 4008. Part 1 explains the repayment terms: the creditor, the amount of the debt, the annual percentage rate, any collateral, whether the creditor says the debt is nondischargeable, and the debtor's income and expenses. Part 2 is a signature certifying that the attached agreement is a true and correct copy.
This page explains the form in plain words and is not legal advice. You can fill it out free in your browser on this page and download the result without an account.

Whether collateral secures the debt, with a description and its current market value, and whether the creditor asserts the debt is nondischargeable. If yes, the form asks for an attached explanation of the nature of the debt and the basis for that contention.
Two columns. Lines 6a to 6d use line 12 of Schedule I (Official Form 106I) and line 22c of Schedule J (Official Form 106J) to reach scheduled net monthly income. Lines 6e to 6h use the amounts stated on the reaffirmation agreement to reach present net monthly income.
Whether the income or expense amounts differ between the two columns, and whether net monthly income on line 6h is less than 0. Line 10 is the debtor's certification that each explanation on lines 7 to 9 is true and correct.
Whether an attorney represented the debtor in negotiating the agreement and has executed a declaration or an affidavit to support it. Part 2 certifies that the attached agreement is a true and correct copy, with signature, date, printed name and a box for who signs.

Line 6 asks for combined monthly income from line 12 of Schedule I and monthly expenses from line 22c of Schedule J, then the amounts stated on the reaffirmation agreement. The form says to put a total that is less than 0 in brackets.
If the answer on line 7, 8 or 9 is Yes, the form asks for an explanation and for line 10 to be completed. Line 9 asks how the debtor will make monthly payments on the reaffirmed debt and pay other living expenses. If all answers are No, the form says to go to line 11.
