Form 4506-T (sp): Request for Transcript of Tax Return, Spanish Version

Form 4506-T (sp) is the Spanish version of Form 4506-T, Request for Transcript of Tax Return. The PDF is in Spanish under the title "Solicitud de Transcripción de la Declaración de Impuestos" and is dated June 2023 ("junio de 2023", Rev. 6-2023). Page 1 is the form and page 2 holds its instructions and mailing charts, also in Spanish.

It asks the IRS for tax return information as a transcript: a return transcript, an account transcript, a record of account, a verification of nonfiling, or a transcript of Forms W-2, 1099, 1098 or 5498 data. The IRS About page says transcripts partially mask personal information while financial data stays fully visible. The instructions say taxpayers on a fiscal year file this form to get a return transcript, and that a copy of a return is requested on Form 4506 (sp) instead.

Line 9 asks for dates "en el formato en español", in the order dd/mm/aaaa. The English edition, Form 4506-T, has its own page. You can fill out this Spanish PDF free in your browser on this page and download it without an account.

What's on Form 4506-T (sp)

Lines 1a to 2b: names and numbers

The name shown first on the return and its SSN, ITIN or EIN, then the spouse's name and number for a joint return.

Lines 3 to 5: addresses and file number

The current address, the address on the last return if different, and an optional customer file number that prints on the transcript.

Line 6: transcript type

One tax form number per request, then a box for a return transcript (6a), an account transcript (6b) or a record of account (6c).

Lines 7 and 8

Verification of nonfiling, which is IRS proof that no return was filed for the year, and transcripts of Form W-2, 1099, 1098 or 5498 data.

Line 9: year or period

Date spaces for the end of each tax year or period requested, written in the Spanish order dd/mm/aaaa.

Signature area

A box confirming the signer has authority, the signature and date, the phone number of the taxpayer on line 1a or 2a, a title line for entities, and the spouse's signature.

How to fill out Form 4506-T (sp)

  1. 1

    Lines 1a and 1b

Enter the name shown first on the return. On 1b enter the EIN for a business return, otherwise the first SSN or ITIN on the return. For a Form 1040 with Schedule C, the instructions say to enter your SSN.

  2. 2

    Lines 3 and 4

Enter your current address, including a P.O. box if you use one. Fill in line 4 only if the last return showed a different address. If they differ and the IRS was not told, the instructions name Form 8822 or Form 8822-B.

  3. 3

    Line 5

Optional. Enter up to 10 numeric characters, never an SSN. The instructions say an SSN or name here makes the transcript show the generic entry 9999999999.

  4. 4

    Lines 6 to 8

Enter one tax form number, such as 1040, 1065 or 1120, and check the transcript you need. The instructions say to ask for the record of account if you are unsure.

  5. 5

    Line 9

Enter the end date of each year or period as dd/mm/aaaa. The form's example is 31/12/2018 for a calendar year 2018 Form 1040. List each quarter for quarterly returns.

  6. 6

    Check the box and sign

Check the box in the signature area confirming your authority; the instructions say the form will not be processed and will be returned if the box is not checked. Sign only after every applicable line is complete.

Where to send it, timing and limits

120-day rule

The form says the IRS has to receive it within 120 days of the date the taxpayer signed it, or it will be rejected.

Where to send it

Mail or fax it to the address for the state where you lived, or where your business was, when the return was filed. One chart covers individual transcripts and one covers all others. If you ask for more than one transcript and the chart shows two addresses, use the one for the address on your most recent return.

Chart addresses

Internal Revenue Service, RAIVS Team at Stop 6716 AUSC, Austin, TX 73301 (fax 855-587-9604); Stop 6705 S-2, Kansas City, MO 64999 (fax 855-821-0094); or P.O. Box 9941, Mail Stop 6734, Ogden, UT 84409 (fax 855-298-1145).

Address of record

The form says that since July 2019 the IRS mails transcripts only to the taxpayer's address of record, not to third parties. A third party that cannot accept that can use the IVES program.

Processing and availability

Most requests are processed within 10 business days. Return transcripts cover the current year and returns processed in the three prior processing years. W-2 and similar data can go back up to 10 years.

Faster options

The form points to "Ordene su transcripción" at www.irs.gov/espanol and to 1-800-908-9946.

Who fills out Form 4506-T (sp)?

  • Individual taxpayers

For a joint return, the instructions say transcripts can go to either spouse and only one signature is needed. Sign exactly as the name appears on the original return.

  • Corporations

An officer with legal authority to bind the corporation, a person the board designates, or any officer or employee on written request of a principal officer, certified by the secretary or another officer. A shareholder of record owning 1 percent or more may also file, with documentation.

  • Partnerships

The instructions say anyone who was a member of the partnership during any part of the tax period on line 9 can generally sign.

  • Representatives

A representative signs only if the taxpayer delegated that authority on line 5 of Form 2848 (sp), and attaches that Form 2848 (sp).

  • Estates, trusts and heirs

The instructions point to section 6103(e) when the taxpayer died or a fiduciary acts for them. An heir, next of kin or beneficiary has to show a material interest, and entities attach the authorizing document.

  • Not for a copy of a return

The instructions say to use Form 4506 (sp), Request for Copy of Tax Return, for copies of returns.

The taxpayer named on line 1a or 2a, or a person authorized to get the information, completes and signs it and sends it to the IRS by mail or fax. The IRS mails the transcript to the taxpayer's address of record.
